The HIPAA privacy policies HPP 7.4 Personal Representative Request and Attachments A and B were revised on Sept. 17, 2024. 

You can view or download the most recent version of the policy on the HIPAA Privacy and Security webpage. 

You can view or download a version of the policy that highlights the revisions on the Office of Institutional Compliance’s Newly Updated Policies webpage.
